External reviews are required where something somewhere has gone wrong and you need an independent, objective person to help spot the factors which caused the issue to arise. There is a multitude of reasons why an issue may have occurred, and we will help you figure out where the issue has occurred and present recommendations around how to mitigate the chance of the issue happening again.

What reviews do we typically investigate?
As trained lawyers, our consultants have a meticulous approach, guided by a deep understanding of ethical standards and ethical issues. Our investigative mindset ensures a thorough ethics review, especially when an organisation faces challenges. We excel at conducting objective and informative ethical reviews that provide clear solutions without overwhelming you.
Each review concludes with actionable recommendations tailored to your needs, helping your organisation meet ethics approval and prevent future issues. We collaborate closely with ethics committees to ensure your practices align with the highest ethical standards. For further information, we provide expert guidance throughout the process.
We were commissioned to undertake an investigation into a series of incidents which occurred within an organisation. There had been a series of serious data breaches which were suspected to have occurred from a defective process. We came in as independent advisors to undertake an external review of the organisation’s policies and procedures, focusing on the incidents and investigating how the incident was even possible to occur. We interviewed several members of staff, including team leads and directors to gain their understanding of the current processes in place.
In the course of our investigations, we found a series of defective processes and a lack of engagement by staff with the existing processes. We produced detailed findings to senior stakeholders, alongside a series of recommendations of how the processes could be changed and improved to mitigate the likelihood of an incident happening in the future.
The organisation was so pleased with our work, that they commissioned us to assist in developing the recommended changes and be a part of their ongoing data protection support to ensure compliance.
Ultimately, every case is different, and we will tailor our investigation to best handle each case. Notwithstanding the above, we have a general approach to cases which involves the following:
1) First, we need to fully understand the issue. We will review all the relevant materials and have discussions with all the relevant people pertinent to the issue.
2) After we have an understanding of the issue, we will cross-reference the facts of the case against the policies and procedures which ought to have been in place to analysis where procedure was or was not followed.
3) After we have ascertained all the facts and understood where the issues are, we will draft a detailed report of the case, explaining our investigation, the issues we picked up and any recommendations and actions we have to mitigate the issue from never happening again, or, how to improve practices at your organisation.
