What reviews do we typically investigate?
Ultimately, every case is different, and we will tailor our investigation to best handle each case. Notwithstanding the above, we have a general approach to cases which involves the following:
1) First, we need to fully understand the issue. We will review all the relevant materials and have discussions with all the relevant people pertinent to the issue.
2) After we have an understanding of the issue, we will cross-reference the facts of the case against the policies and procedures which ought to have been in place to analysis where procedure was or was not followed.
3) After we have ascertained all the facts and understood where the issues are, we will draft a detailed report of the case, explaining our investigation, the issues we picked up and any recommendations and actions we have to mitigate the issue from never happening again, or, how to improve practices at your organisation.
